Office Hours: Our office hours are Monday through Thursday from 8:00 A.M. to 5:00 P.M.

Parking: Parking is available for patients.

Confidentiality:  Dental information is released only with your consent.

Payment: Payment is due at each visit.  We accept cash, checks, and most major credit cards.  If you can't pay at the office, please talk to the business manager and arrange for payment within 30 days. 

A fee schedule for  crowns, bridges, dentures or other major dental work will be established before treatment begins.  We understand those costs can add up.  Don't ignore bills you can't pay.  Ask us to help you set up a reasonable plan.

If you are covered by dental insurance, we'll fill out the forms.  Just sign your part so we can release your records.

Insurance may pay all, some or none of your bill, but you are responsible for paying any charges not covered by your insurance.

There is no charge for completion of the first claim form for any visit.  You should direct any questions or complaints regarding your coverage to your insurance carrier.
